If you've decided to come to our facility for therapy, first, thank you for choosing us for your therapy needs! Here is what you can expect on your first visit. First, you need to call the office to schedule your initial evaluation. When you come in for that visit, we will need: - Your insurance card - Claim information for auto accident and worker's comp cases - Your drivers license or ID - Intake paperwork (most of which can be downloaded below) You don't have to fill out your paperwork before you come, but it will save you some time. Martha Bohon "I started with knee pain, and was here for therapy to improve that condition. While working on the knee, I also developed shoulder problems. We added shoulder therapy to the regimen. After knee surgery, I was back to therapy. I am happy to say that my knee is back to 100%.
